Sorbifer Durules- antianemic drug. Iron is an indispensable component of the body, necessary for the formation of hemoglobin and the occurrence of oxidative processes in living tissues.

Durules technology provides a gradual release of the active ingredient (iron ions) over a long time. The plastic matrix of Sorbifer Durules tablets is completely inert in the digestive juice, but completely disintegrates under the action of intestinal peristalsis when the active ingredient is completely released.

Ascorbic acid improves iron absorption.

Compound

Iron sulfate + Ascorbic acid (vitamin C) + excipients.

Pharmacokinetics

Durules is a technology that provides a gradual release of the active substance (iron ions), a uniform flow of the drug. The intake of 100 mg 2 times a day provides a 30% greater absorption of iron from the drug Sorbifer Durules compared to conventional iron preparations. Absorption and bioavailability of iron are high. Iron is absorbed predominantly in the duodenum and proximal jejunum. Plasma protein binding - 90% or more. It is deposited in the form of ferritin or hemosiderin in hepatocytes and cells of the system of phagocytic macrophages, a small amount - in the form of myoglobin in muscles.

Pharmacological properties

Pharmacokinetics

"Durules" is a special production technology that provides a gradual release of the active substance (iron ions), a uniform flow of the drug. Iron is absorbed mainly in the duodenum and proximal jejunum. Taking 100 mg twice a day provides a 30% greater absorption of iron from Sorbifer Durules compared to conventional iron preparations. Absorption and bioavailability of iron are high. Communication with plasma proteins - 90% or more. It is deposited in the form of ferritin and hemosiderin in hepatocytes and cells of the system of phagocytic macrophages, a small amount - in the form of myoglobin in the muscles. The half-life is 6 hours.

The presence of ascorbic acid in the preparation creates more favorable conditions for the absorption of iron in the intestine. At the molecular level, ascorbic acid mobilizes iron from the crystalline core of ferritin. in vitro, reducing Fe 3+ to Fe 2+ . At the intracellular level, ascorbic acid enhances iron-induced translation of ferritin by promoting the conversion of iron regulatory protein (IRP) from RNA-bound form to aconitase.

Pharmacodynamics

Iron is an indispensable component of the body, necessary for the formation of hemoglobin and the implementation of oxidative processes in living tissues. The active substance is contained in a biologically indifferent plastic matrix of a spongy structure. When passing through the gastrointestinal tract from the porous matrix of tablets (within 6 hours), there is a continuous release of ferrous ions. The film coating of the tablet and the porous matrix provide a long-term release of iron ions. Prevention of dissolution of the tablet in the stomach contributes to the film coating of the tablet, which completely disintegrates under the action of intestinal peristalsis, when the active ingredient is released. The slow release of iron ions does not lead to a high local concentration, which avoids irritation of the gastrointestinal mucosa.

Ascorbic acid slows down the breakdown of ferritin by blocking the autophagy of ferritin by ferritin lysosomes and the transformation into hemosiderin.

Ascorbic acid accelerates the absorption of iron in the gastrointestinal tract, restoring unbound heme iron (III) to iron (II) in the stomach.

Indications for use

Iron-deficiency anemia

Latent iron deficiency in the body (without anemia) associated with excessive iron loss (bleeding, including uterine bleeding, constant donation), malnutrition

Conditions accompanied by an increased need for iron in the body (prevention during pregnancy, lactation and in blood donors)

Dosage and administration

The tablet should be taken whole, without chewing, and at least half an hour before a meal, with half a glass of water.

Adults and teenagers over 12 years of age:

For patients with iron deficiency anemia II-III degree, if necessary, on the recommendation of a doctor, the dose can be increased to 3-4 tablets 2 times a day (morning and evening) with a duration of use for 3-6 months.

During pregnancy:

The drug is used in case of established iron deficiency (with iron deficiency anemia and latent iron deficiency in the body)

Prophylactic dose: 1 tablet per day.

Therapeutic dose: 1 tablet 2 times a day (morning and evening).

Treatment should be carried out until the hemoglobin level normalizes and continue until the iron depot is completely saturated for another 2 months. Individual long-term therapy (with or without interruptions) is indicated for regularly occurring significant iron loss (for example, with heavy menstruation).

Side effects

The frequency of side effects from the digestive system increases with increasing doses from 100 to 400 mg per day.

Often (>1/100)

Nausea, abdominal pain, diarrhea, constipation

Rarely (<1/100)

- esophageal ulcer, esophageal stenosis

Allergic reactions (itching, rash, hyperemia)

hyperthermia

Contraindications

Hypersensitivity to the active or any other inactive component of the drug

Esophageal stenosis and/or other obstructive changes in the digestive tract

Increased iron content in the body (hemosiderosis, hemochromatosis)

Repeated blood transfusions

Impaired iron utilization (lead anemia, sideroblastic anemia, hemolytic anemia)

Children under 12 years of age (due to lack of clinical data)

Drug Interactions

- ciprofloxacin: simultaneous administration reduces the absorption of ciprofloxacin by approximately 50%, so that there is a risk that the content of ciprofloxacin in the blood plasma will be lower than necessary for a therapeutic effect.

- levofloxacin: simultaneous administration reduces the absorption of levofloxacin.

-moxifloxacin: simultaneous administration reduces the bioavailability of moxifloxacin by almost 40%, therefore, if simultaneous use is required, it is necessary to ensure the longest possible period between taking moxifloxacin and Sorbifer Durules.

- norfloxacin: simultaneous administration reduces the absorption of norfloxacin by approximately 75%.

- ofloxacin: simultaneous administration reduces the absorption of ofloxacin by approximately 30%.

With the simultaneous use of the drug Sorbifer Durules with the following drugs, dosage adjustment of these drugs may be required. The recommended minimum time interval between taking Sorbifer Durules and these drugs should be at least 2 hours:

- calcium or food supplements containing magnesium carbonate, as well as aluminum hydroxide or calcium, antacids containing magnesium carbonate, together with iron salts form a complex that reduces the absorption of each other.

- captopril: Co-administration reduces the area under the plasma concentration-time curve for captopril by approximately 37%, presumably due to chemical reactions in the gastrointestinal tract.

- zinc: simultaneous use reduces the absorption of zinc salts.

- clodronate: simultaneous use may reduce the absorption of clodronate.

- deferoxamine: with simultaneous use, the absorption of deferoxamine and iron decreases, due to the formation of the compound.

- levodopa: with simultaneous use of ferrous sulfate reduces the bioavailability of single doses of levodopa by about 50%, and single doses of carbidopa by 75%, possibly due to the formation of a chelate compound.

- methyldopa: With simultaneous use, the bioavailability of methyldopa is reduced, possibly due to the formation of a chelate compound.

- penicillamine: the simultaneous use of penicillamine and iron salts reduces their absorption, possibly due to the formation of a chelate compound.

- risedronate: held in vitro studies have shown that preparations containing iron form compounds with risedronate. Although drug interaction studies have not been conducted in vivo, it can be assumed that the simultaneous use of these drugs reduces the absorption of risedronate.

- tetracyclines: with simultaneous use, the absorption of tetracyclines and iron decreases. If simultaneous administration is necessary, the recommended minimum time interval between taking Sorbifer Durules and these drugs should be at least 3 hours. When taken orally, iron inhibits the enterohepatic circulation of oxytetracixin (doxycycline), as well as when administered intravenously.

- thyroid hormones: with the simultaneous use of iron and thyroxine preparations, a decrease in the absorption of thyroxin is possible, which reduces the effectiveness of replacement therapy.

-cimetidine: with simultaneous use, the decrease in gastric acid production caused by cimetidine reduces the absorption of iron. Therefore, the intervals between taking these drugs should be at least 2 hours.

-chloramphenicol: when taken simultaneously, it is possible to delay the clinical effect of iron treatment.

While taking the drug with tea, coffee, eggs, dairy products, wheat flour, porridge or food rich in vegetable fibers, iron absorption may be reduced.

special instructions

Iron preparations can cause poisoning in children. When using the drug, darkening of the stool appears, which has no clinical significance.

The drug is used with caution in gastric and duodenal ulcers, inflammatory bowel diseases (enteritis, diverticulitis, ulcerative colitis, Crohn's disease), chronic liver and kidney diseases.

Comprehensive laboratory and instrumental monitoring of the effectiveness of treatment is recommended every 7-14 days, depending on the course of anemia.

Pregnancy and lactation

The drug can be used during pregnancy and lactation.

Features of the effect of the drug on the ability drive a vehicle or potentially dangerous machinery.

Does not affect.

Overdose

Symptoms: abdominal pain, vomiting and diarrhea (sometimes with blood), fatigue, weakness, hyperthermia, paresthesia, pallor of the skin, cold clammy sweat, acidosis, weak pulse, lowering blood pressure, palpitations. Signs of peripheral circulatory collapse, coagulopathy, hyperthermia, hypoglycemia, liver damage, renal failure, muscle cramps, and coma may occur after 6 to 12 hours.

Treatment: gastric lavage, inside milk and a raw egg, means that provoke vomiting, symptomatic therapy.

If necessary, make a gastric lavage with a solution of deferoxamine at a concentration of 2 g / l, then 5 g of deferoxamine is dissolved in 50-100 ml of water and this solution is left in the stomach.

In severe intoxication: in a state of shock and / or coma and in the event of an increase in serum iron levels (> 90 mmol / l in children, > 142 mmol / l in adults), intensive therapy should be started immediately and deferoxamine should be administered (15 mg / l kg/h IV slowly, maximum 80 mg/kg/24 hours). Too high an infusion rate can lead to hypotension.

In less severe cases of intoxication, deferoxamine can be administered intramuscularly (50 mg/kg, total dose not more than 4 g).

Sorbifer Durules

Sorbifer Durules- an antianemic combined drug, which combines ferrous sulfate with ascorbic acid. The pronounced therapeutic effect of the drug in anemia (anemia) is ensured precisely due to the combined action of its components: ferrous sulfate replenishes iron deficiency in the body, and vitamin C increases its absorption in the digestive tract.

Durules- a special manufacturing technology of the drug, which allows iron ions to be evenly and gradually released in the intestine not under the action of digestive juices, but as a result of peristalsis (wave-like movements of the intestinal walls).

Such a gradual (over 6 hours) release of iron ions prevents the creation of an increased amount of them in the digestive organs and prevents the irritating effect of iron ions on the mucous membrane of the digestive tract.

The absorption of the drug in the small intestine, thanks to this technology, is high, 30% higher than that of others iron preparations. It is Ascorbic acid that increases its absorption.

Release form

Coated tablets of 30 and 50 pieces in a vial. 1 tablet of Sorbifer Durules contains 100 mg of iron and 60 mg of vitamin C.

Instructions for use Sorbifer Durules

Indications for use

  • Treatment and prevention of iron deficiency anemia caused by various causes;
  • prolonged, heavy bleeding (uterine, gastrointestinal, nasal);
  • replenishment of iron deficiency with an increased need for it (after serious illness, during breastfeeding, during pregnancy, intensive growth in adolescence, with constant donation);
  • replenishment of iron deficiency in violation of its absorption (with prolonged diarrhea);
  • insufficient, malnutrition.

It is advisable to use Sorbifer Durules only in case of iron deficiency anemia. With anemia caused by another reason, the drug should not be prescribed.

Before starting treatment, iron-binding activity and the level of iron in the blood are determined.

Contraindications

1. High iron content:
  • hemochromatosis (hereditary disorder of iron metabolism);
  • hemosiderosis (a disease associated with increased breakdown of red blood cells).
2. Diseases associated with impaired absorption of iron:
  • aplastic anemia (inhibition of bone marrow hematopoiesis);
  • hemolytic anemia (associated with increased destruction of red blood cells);
  • lead anemia (occurs with lead poisoning);
  • sideroblastic anemia (the level of iron is sufficient, but the body cannot use it);
3. bleeding;
4. Condition after removal of the stomach;
5. Age up to 12 years;
6. Narrowing of the lumen of the esophagus or other parts of the digestive tract;
7. Intolerance to any component of the drug.

Sorbifer should be used with caution in Crohn's disease (inflammatory process of various parts of the digestive tract), diverticulitis, peptic ulcer (12 duodenal ulcer, stomach or large intestine), inflammation of the small intestine (enteritis).

Side effects

During treatment with Sorbifer, vomiting, abdominal pain, nausea, an unpleasant aftertaste in the mouth, stool disorders (constipation or diarrhea), loss of appetite may occur. These manifestations may increase with increasing doses up to 400 mg.

Extremely rarely, such adverse reactions as ulceration of the esophagus, narrowing of the esophagus, headache, fever of the skin, weakness, dizziness, skin allergic manifestations (rashes, skin itching) are observed.

Treatment with Sorbifer

How to take Sorbifer?
The tablet should be swallowed whole, without chewing, half an hour before meals; take the tablet with water (at least 100 ml).

Against the background of treatment with Sorbifer, as well as other iron-containing drugs, there is a black color of feces. Its appearance should not cause concern to the patient.

Reduce the absorption of Sorbifer egg yolk, tea, dairy products.

Dosage of Sorbifer
The usual therapeutic dose of Sorbifer is 1 tablet or 100 mg 2 times a day. With the appearance of side effects, the dose of the drug is reduced to 1 tablet per day.

Depending on the severity of anemia, the dose may be increased to 300-400 mg per day (3-4 tablets) in 2 divided doses.

The duration of the course of treatment is determined by the doctor. During treatment, a complete blood count and serum iron levels are monitored. After normalization of indicators, treatment is usually extended by 2 months to create a supply of iron in the body. In severe anemia, the course can be extended up to 3-6 months.

The prophylactic and maintenance dose is 100 mg (1 tablet) per day.

The simultaneous use of Sorbifer with other iron-containing drugs should be avoided (to avoid overdose). In case of an overdose, the following symptoms may appear: vomiting, abdominal pain, diarrhea with blood, weakness, pallor of the skin, lowering blood pressure, weak pulse, palpitations, fever, smell of acetone from the mouth. In severe cases of overdose, kidney and liver failure, low blood sugar, convulsions and loss of consciousness may occur.

Sorbifer for children

Sorbifer Durules can be prescribed to children over 12 years of age at a daily dosage of 3 mg per 1 kg of body weight.

Clinical data on the use of Sorbifer in children under 12 years of age are not available.
